Le porfirine sono un gruppo di composti definiti dalla loro struttura chimica. Questi composti sono sottoprodotti della sintesi dell’eme e sono normalmente presenti in basse concentrazioni nel sangue e nei fluidi corporei. L’esame misura le porfirine ed i loro precursori nell’urina nel sangue e/o nelle feci.Il gruppo eme è un pigmento contenente ferro che compone l’emoglobina e numerose altre proteine. È costituito da una porzione organica (protoporfirina) legante un atomo di ferro. La sintesi dell’eme é un processo graduale che richiede l’azione sequenziale di 8 enzimi diversi. La carenza di uno di questi enzimi interrompe tale processo, con conseguente accumulo dei precursori della porfirina, quali uroporfirine, coproporfirine e protoporfirine, nei fluidi e nei tessuti dell’organismo. Il tipo di precursori che si accumulano dipende dall’enzima carente o non funzionale; i precursori possono essere tossici.Il test consente di diagnosticare e monitorare un gruppo di patologie definite porfirie. Esistono sette tipi di porfirie, ciascuno dei quali è associato ad una diversa carenza di enzimi. La maggior parte delle porfirie sono ereditarie, dovute alla presenza di variazioni nella sequenza di un gene. Le porfirie vengono classificate come neurologiche, cutanee o entrambe, sulla base dei segni e sintomi presenti.Le porfirie con sintomi neurologici si presentano con attacchi acuti che possono durare per giorni o settimane. Sono caratterizzate dalla presenza di dolori addominali, costipazione, stato confusionale, allucinazioni e convulsioni. Esistono quattro tipi di porfirie neurologiche: Porfiria Acuta Intermittente (AIP), Porfiria Variegata (VP), Coproporfiria Ereditaria (HCP) e Porfiria ALA-Deidratasi Carente (ALAD), particolarmente rara. In alcuni casi di VP e di HCP possono essere presenti anche sintomi cutanei.Le porfirie cutanee sono associate a fotosensibilità, che causa rossore, gonfiore, sensazione di bruciore, vescicole, ispessimento della pelle, iperpigmentazione ed in alcuni casi cicatrici. Esistono tre tipi di porfirie cutanee: Porfiria Cutanea Tarda (PCT), Protoporfiria (anche detta Protoporfiria Eritropoietica; EPP) e Porfiria Eritropoietica Congenita (CEP).Attualmente la comunità scientifica non concorda sull’eventualità che tutte le porfirie siano ereditarie. La porfiria cutanea tarda (PCT) può essere determinata da fattori genetici o ambientali, come l’esposizione a determinate sostanze chimiche, o dalla presenza di lesioni significative a livello epatico. Questo tipo di PCT viene definito “acquisito” o “sporadico”. Nei pazienti con PCT acquisita la patologia si sviluppa solitamente dopo i 30 anni, mentre l’esordio in età infantile è raro.La diagnosi di queste patologie si avvale della misura delle porfirine e dei loro precursori nell’urina, nel sangue e/o nelle feci. Il test può includere la misura di uno o più dei seguenti composti:
- Porfobilinogeno (PGB), un precursore della porfirina presente nell’urina
- Acido delta-aminolevulinico (ALA) un precursore della porfirina presente nell’urina
- Porfirine (uroporfirine, coproporfirine e protoporfirine) nell’urina, nelle feci e nel sangue
