Gli anticorpi anti-cellule parietali sono autoanticorpi prodotti dal sistema immunitario e in grado di riconoscere ed attaccare le cellule specializzate della parete dello stomaco dell’organismo di appartenenza. Questo esame consente di rilevare gli anticorpi anti cellule-parietali.L’anemia perniciosa è una malattia autoimmune caratterizzata dalla produzione di autoanticorpi in grado di riconoscere e attaccare le cellule parietali e/o il fattore intrinseco.

  • Le cellule parietali sono cellule presenti nello stomaco deputate alla produzione dei succhi gastrici e del fattore intrinseco e quindi in grado di favorire la digestione.
  • Il fattore intrinseco è una molecola necessaria per l’assorbimento della vitamina B12 dagli alimenti.

Durante la digestione, i succhi gastrici prodotti dalle cellule parietali, consentono il rilascio della vitamina B12 dagli alimenti: La vitamina B12 viene quindi legata dal fattore intrinseco a formare un complesso che consente poi l’assorbimento della vitamina nell’intestino tenue. La vitamina B12 è essenziale per il corretto funzionamento dell’encefalo e del sistema nervoso, ed è anche implicata nella produzione dei globuli rossi (RBC).Nella gastrite atrofica autoimmune, gli autoanticorpi attaccano le cellule parietali e/o il fattore intrinseco, con la conseguente instaurazione di un processo infiammatorio progressivo. Ne consegue la distruzione delle cellule parietali e la carenza di fattore intrinseco.La carenza di fattore intrinseco determina il mancato assorbimento e quindi la carenza di vitamina B12. Questa può portare ad anemiamegaloblastica (anemia perniciosa), con presenza di RBC più grandi del normale (macrociti) ma in numero ridotto. Possono essere presenti anche sintomi neurologici (neuropatia), come intorpidimento e formicolio delle mani e dei piedi, debolezza muscolare, rallentamento dei riflessi, perdita della stabilità durante la camminata.In queste patologie, oltre all’anemia, possono esservi neutropenia (carenza di neutrofili) e trombocitopenia (carenza di piastrine).Questo esame può essere richiesto insieme ad altri esami, come l’emocromo e lo striscio di sangue periferico.
