DHEA derives almost exclusively from the zona reticularis of the adrenal cortex; it can also be produced by the testes but not by the ovaries; the serum concentration of DHEA is 1000 times lower than that of DHEAS and, compared to the latter, presents marked circadian variations. Weakly androgenic, DHEA is metabolised, at the peripheral tissue level, mainly to testosterone and dihydrotestosterone. Serum DHEA levels increase progressively during childhood and adolescence, increase rapidly after puberty and reach a peak at approximately age 20 and then progressively decrease.
